This patient group direction (PGD) template supports the administration of diphtheria, tetanus and inactivated poliomyelitis vaccine (Td/IPV), to individuals from 10 years of age, in accordance with national immunisation programme, for travel or for the management of cases and contacts of diphtheria, tetanus or poliomyelitis.

Td/IPV(Revaxis®)PGDV03.00 is valid from 1 November 2019 to 31 October 2021.

Practitioners must not use thisPGDtemplate until it has been authorised in Section 2. This is a legal requirement (seeHuman Medicines Regulations 2012). Practitioners should follow local policy and procedures to access authorisedPGDdocuments.

ThisPGDtemplate should be used with reference to current national guidance, theGreen Book, andSummary of Product Characteristicsfor the vaccine.
